Please complete the form below to request a discounted shipping label and pickup date for your blankets.  Once we receive your request, your label will be sent to you via email or regular mail depending on your selection below.  Also, in a separate email, you will receive a service order form with instructions.  Please include this completed order form with your blanket shipment.  DO NOT SEND WET BLANKETS!!  A surcharge will be added for blankets that are received wet. 

The cost for shipping the blankets to us will be added to your final invoice.  (Because we ship in volume, the cost is less than what you would pay directly.)  As always, ECLS pays for the return shipping.  If you have any further questions or concerns, please contact us.
